Student Health Insurance

This website has been created to provide students of Pacific Graduate School of Psychology with information and guidance on the PGSP Student Health Insurance Plan. Please visit our FAQ Page.
Information on this site is for the 2008-09 policy year except where noted otherwise.
PGSP has instituted a mandatory health insurance requirement for all full-time and part-time matriculating students.
Those students who are registered and attending classes are eligible for and are automatically enrolled and insured under this plan, unless they timely complete a waiver form identifying their own health insurance. Students must complete the online waiver form with the student’s own health insurance information. If you have problems or questions filling out the online waiver form please contact support@pgsp.edu or call the helpdesk at 650-421-4839.
Students enrolled in a Distance Learning program are not eligible for coverage under this plan.
PGSP will automatically enroll and charge you for the student health insurance unless an acceptable waiver is submitted.
DEADLINE for Submission of Online Waiver for Winter 2009 : DECEMBER 2, 2008
IMPORTANT – It is every student's responsibility to understand the insurance requirements.
IF YOU WAIVED COVERAGE FOR FALL 2008 AND YOU NOW WISH TO GET COVERAGE PLEASE COMPLETE THE ON-LINE APPLICATION FORM
To Enroll for Coverage:
1) Coverage is automatic for students unless a waiver form was submitted
2) Go to the online application form
3) Click on the application link
4) Complete information and click on Save button
5) Click on E-mail Form button
Note: To enroll dependents, please complete the enrollment form found in the Health Plan Brochure.
To Waive Coverage
1) Go to the online waiver form
2) Click on the waiver link
3) Complete information on form and click on Save button.
4) Click on E-mail Form button
5) If you have problems or questions filling out the online waiver form please contact support@pgsp.edu or call the helpdesk at 650-421-4839
6) You may submit waiver via online, fax, or US Mail
There is an issue on the Macintosh where PDF forms will not email by pressing the Email form button. The workaround is below:
1) Fill out the On-line Student Health Insurance Waiver form in Preview.
2) Go to File and select Print.
3) Select PDF and then Save as PDF.
4) Rename the document and save it to your Desktop or Documents folder.
5) Email the saved PDF to studenthealth@pgsp.edu.
